How do you make a referral to the CARE Team?
To make a referral to our CARE Team, simply fill out the online Student of Concern Referral Form. Please include all relevant information and as many details as possible. This form generates a report that is accessed by the CARE Team Coordinator.
Who can make a referral?
Anyone. Students, faculty, staff, and community members are encouraged to complete the Student of Concern Referral Form if they suspect a student is in distress.
When should you refer a student to CARE Team?
If you are concerned about a student, complete the Student of Concern Referral Form. The CARE Team is here to help any student in distress. Common signs of distress include:
- Marked Change in Academic Performance/Behavior
- Unusual Behavior or Appearance
- Emotional or Life Stressors
- References to Death/Suicide
Why should you refer a student to CARE Team?
We believe all students deserve a safe, happy, healthy college experience. Early response to the needs of distressed students can prevent increased risk and retain the student's success in and out of the classroom. Referrals help us keep students and our campus community safe.
In the case of an emergency, CALL 911 for IMMEDIATE ASSISTANCE. Student of Concern Referral Forms are not checked after business hours.
