IRB Submission Quick Steps
*In order to ensure the accuracy of each IRB submission, it is the responsibility of the person(s) submitting for approval that each of the appropriate boxes are checked. A submission will not be reviewed for implementation until the check list is complete. Please include the completed check list with each submission as a table of contents. Check List.

Step 1 – Complete and Submit Training

Step 2 – Submit Research Application and Protocol Form
If you are a UNA Student or Faculty Member you must use your UNA portal email for all communications and use on all applicable forms (application, proposal, consent/assent forms, etc.). -
Step 3 – Complete and Submit any other required documentation such as Assent and Consent Forms (below), copies of surveys, etc.

Step 4 – Submit all forms electronically by self-enrolling on the Canvas website, https://una.instructure.com/enroll/C4YF3L. Once enrolled, locate the module entitled "Submission Portal Module", click on the assignment labeled "IRB proposal submission" and upload all of the documents for the proposal into Canvas.

All forms must be submitted through a university established email account.
