How to Complete Patient Portal Forms on your Cell Phone

  1. Visit this link.

  2. Sign in with your UNA Portal credentials.

  3. Click the menu bar (grey bar with three blue lines).

  4. Click on “Forms”.

  5. Scroll down to “UNA Patient Forms” and complete the following forms: UNA Medical History, UNA Privacy Policy Acknowledgement, UNA Release Authorization, and all other forms related to your visit (UNA Physical Form for Physicals, STI Test Consent Form for Sexual Health/UTI Visits, TB Questionnaire for TB Skin Tests, Women’s Health Form for Women’s Health Visits, etc.)

  6. Go to “Messages” for Class Excuses. Go to “Statements” for receipts from your visit.
