CPH Alleged Infraction Report

CPH Alleged Infraction Reports must be completed and submitted to the College Panhellenic peer accountability officer or fraternity/sorority advisor within 30 calendar days from the date of the alleged infraction (including university breaks).

Non-recruitment infractions may be reported by one of the following:

  • Chapter president
  • Chapter executive officer/board member
  • CPH officer
  • Fraternity and Sorority Life Office Staff 

Recruitment infractions may only be reported by one of the following:

  • Chapter president
  • CPH Director of Recruitment
  • Recruitment Counselor
  • Potential new member
  • Fraternity and Sorority Life Office Staff

Chapter officers should list their organization name. CPH Exec and Recruitment Counselors should list CPH.
List specific NPC Unanimous Agreement, NPC policy, CPH bylaws, CPH recruitment rule, etc.
Please include name, affiliation, officer title, email addresses, and phone numbers.

Please make sure all required fields have been filled out correctly.
Thank you for submitting this report. The report will be reviewed by the CPH President and advisor. You will be notified if your report is not properly submitted. If the report is submitted properly, a copy of the report will be sent to the accused sorority, the FSA, and the NPC Area Advisor within seven days.
