Students may also click the forms tab in the patient portal to complete all health forms. The forms listed with a red asterisk are a requirement that should be completed BEFORE coming to Health Services for an appointment. These forms should also be updated annually.
UNA Immunization Requirements:
MMR: Measles, Mumps, Rubella- Students born after 1956- 2 doses of MMR at least 28 days apart after 12 months of age OR a copy of a lab report showing proof of immunity from Measles (Rubeola), Mumps, and Rubella can be submitted.
VARICELLA – (Chickenpox/Shingles) – Students born after 1979 – 2 doses of Varicella vaccine at least 28 days apart OR healthcare provider documented history with the date of the disease OR a copy of a lab report showing proof of a positive Varicella IgG antibody.
MENINGOCOCCAL - Meningitis (A, C, W, Y) (Menactra or Menveo) - Students age 21 and younger – Proof of immunization against meningitis given any time after the student’s 16th birthday.
TDAP - Tetanus, Diphtheria, Acellular Pertussis- All Students - One dose within the past 10 years. Booster required if vaccine greater than 10 years old.
TUBERCULOSIS SCREENING QUESTIONNAIRE: All students MUST complete the Tuberculosis Screening questions. Further tuberculosis testing may be required based upon information received from the screening questions. *A student with a positive TB screening test cannot attend class until follow-up testing determines there is no active Tuberculosis disease. *All TB testing must be performed in the United States.
MEDICAL HISTORY FORM: All students must complete the Medical History Form. This form should be updated annually.
