Space Request Form

This document serves as a tool to assure equitable use of space within the Department of Music. Remember to consult the daily printed list before inquiring which spaces are available. Please complete this form to reserve any space within the department. Room 101 (band room) is not present in the space reservation system and will require special permission from Dr. Lloyd Jones, Director of Bands and Dr. Terrance D. Brown, Interim Chair. 

Students MUST receive approval from the applied instructor, faculty advisor, etc. before attempting to reserve any space. SPACE REQUEST FORMS MUST BE SUBMITTED TO THE MUSIC DEPARTMENT OFFICE A MINIMUM OF 72 HOURS IN ADVANCE.
