COVID-19 Self-Screening Assessment: In accordance with the recommendations of the Centers for Disease Control and Prevention (CDC), UNA will implement a return to work process to safely return employees to campus. The CDC recommends recall of employees by category based on their degree of exposure to the illness. Human Resources, department leaders, and University Health Services (UHS), are working together to implement the campus self-screen and monitoring plan. Employees will need to review the COVID-19 Self-Screening Assessment below before returning to campus and daily thereafter, until further notice. In your review of the questions below, if you feel that you should answer with a "yes", you must call UHS at 256.765.4328 to determine if continued isolation from campus is indicated. UHS will provide further instructions via phone consultation. The questions below are based on guidance provided by the CDC and the Alabama Department of Public Health (ADPH) and are subject to change.
