  • 1. Who must submit proof of immunization? All students admitted to UNA for the first time must submit proof of immunization via Med+Proctor
  • 2. What immunizations are required? The immunization requirements are listed on our webpage: Health Services Website
  • 3. How do I submit proof of immunization? All students must register for an account using their UNA email address on Med+Proctor. An account can be created as soon as the student receives their UNA email address.
  • 4. When should I submit proof of immunization? Immunization information and any supporting documentation should be submitted as soon as a Med+Proctor account is created
  • 5. How will I know if my documentation meets UNA’s immunization requirements? Med+Proctor will correspond with students through their UNA email. Please remember to check junk & spam folders if you have not received any correspondence.
  • 6. I am admitted to UNA, but I have not attended class for a while. Do I have to meet the immunization requirements and submit an immunization form? Students admitted to UNA for the first time are required to provide proof of immunity. University Health Services advises all students to be properly immunized.
  • 7. I am taking one class on campus, do I have to meet the immunization requirements and submit an immunization form? Yes, you are required to complete the immunization form.
  • 8. I am taking online classes. Do I have to meet the immunization requirements and submit an immunization form? Yes, you are required to complete the immunization form. If your classes are online and you will not be on campus, you may request consideration for exemption by waiver
  • 9. What if there is a reason that I cannot meet the immunization requirements? A request for exemption must be submitted for each vaccine requirement that is not met, regardless of the reason. Please consult with your healthcare provider for a medical waiver.
  • 10. Can I call University Health Services if I have a question? The best way to receive an answer to your question(s) is to type immunization question into the subject line and send the question via email to healthservices@una.edu . The Health Services staff are providing care for students and will return non-urgent calls at the end of the day, or as time permits.
  • 11. I cannot find my immunization information, how else can I meet the requirements? The first and best option to show proof of the required immunizations is obtaining your childhood immunization records from your health care provider or the state department of public health. If you are unable to locate your immunization records: a) Receive the vaccination(s) from your local healthcare provider. Many walk- in clinics and pharmacies can provide the required immunizations. b) If you think you received the vaccine at some point in your life, but you cannot locate the documentation, you may choose to obtain a blood test known as a titer. This can be done at your local doctor’s office. The blood test can determine if you are immune to the disease. If the blood test shows that you are immune, nothing more is needed. If the blood test shows that you are not immune, you will need to receive the vaccine.
  • 12. What if I fail to meet the immunization requirements, or I do not submit my information to Med+Proctor? If you have not provided proof of immunization by the first day of class, your status will be non-compliant by default waiver. Students that default into a noncompliance waiver may be asked to leave the campus if there is an outbreak of one of the vaccine preventable diseases.
